Procedure on Requesting for School Records

GENERAL GUIDELINES

  1. Client emails Registrar’s Office at regoffice@ndu.edu.ph for their particular request/needs.
  2. The concerned Registrar’s staff will check /assess the records of the client.
  3. Reply the Client’s email as follows:
    • If the Records are compliant, the client will be given their scheduled visit to the University
    • If the records are not compliant, the client will be advised as to the documents that  she/she must comply before transacting with the office.
  4. The Registrar’s Office will accommodate clients both those who have emailed as well as walk-in clients for as long as they are qualified to go out per ruling of the IATF.
  5. In compliance to RA 10173 on DATA PRIVACY ACT OF 2012, for  school documents requested through a representative, the following must be submitted:
    • A formal letter of Authorization  from the student/graduate
    • Photocopy of the ID of the student/graduate
    • Photocopy of the ID of the authorized representative

 

PROCEDURE:

  1. Secure form “Request for School Records” at the designated window of the Registrar’s Office.
  2. Fill out the form and submit it to the  records officer at the window for assessment.
  3. Pay the assessed amount at the NDU Business Office and return the form with the official receipt to the Registrar’s Office.
  4. Claim the requested school records indicated on the claim stub personally. If records will be picked up by a representative, an authorization letter and a photocopy of the valid ID of the owner of the records and his/her representative must be submitted to the Registrar’s Office.

 

REQUIREMENTS:


Transcript of Records/Certificate of Transfer Credentials (for Graduates):

  1. Original Birth Certificate in NSO/PSA Security Paper (if not submitted yet)
  2. Marriage Certificate for female student/graduate who used her married surname upon admission to Notre Dame University (if not submitted yet)
  3. One (1) pc.  recently taken 2 x 2 picture  with white background (Selfie taken picture will not be accepted)


Transcript of Records/Certificate of Transfer Credentials (Undergraduates):

  1. Original Birth Certificate in NSO/PSA Security Paper (if not submitted yet)
  2. Marriage Certificate for female student who used her married surname upon admission to Notre Dame University (if not submitted yet)
  3. Form 137 
  4. TOR marked “COPY FOR NOTRE DAME UNIVERSITY” for transferees
  5. One (1) pc.  recently taken 2 x 2 picture with white background (Selfie taken picture will not be accepted)

 

Certification Authentication Verification (CAV) / Red Ribbon:

  1. Original copy of Official Transcript of Records
  2. Original Copy of Special Order/Certification of Graduation
  3. Original Copy of Diploma
  4. Original copy of RLE (for BSN graduates)
  5. Special power of Attorney (if NDU will process CAV/Red Ribbon)

 

Authentication of Official Transcript of Records:

  1. Original Copy of Official Transcript of Records in the new OTR format. However, due to an urgent need, authentication of the old transcript (old format)  may be  granted but only once. Subsequent request for authentication requires the original copy in the New OTR format.  Clients are advised to apply for a new OTR.

 

Second Copy of Diploma

  1. Duly-notarized Affidavit (Damage or Loss).

    Note: The second copy of the diploma is issued ONLY ONCE and should the same be damaged or lost thereafter, no request for this document may be further entertained.
